ABSTRACT The Sphenoid Sinus is considered the paranasal sinus with more variation to degree of pneumatization. Thus, this work aimed report first case on interference anatomical for orthognathic surgery. A 18- year-old woman, isolated cleft palate repaired, was submitted surgery correct maxillomandibular skeletal discrepancy. cone beam computed tomography (CBCT) exam performed preoperative planning showed a great extension adjacent structures, proximity posterior wall maxillary and pterygomaxillary fissure bilaterally. postoperative CBCT image evidenced compromise floor due factors related transoperative period. This case, along literature review, highlighted importance identifying sphenoid in exams prior surgery, order avoid serious complications patient, such as infections; hemorrhages; mucocele formation or intracranial involvement.
